Declare as follows: 1. That according to my knowledge all information provided in this application is true and correct, and I declare myself bound to all obligations, undertakings and information it contains or which may result from the DIGITAL MicroCredit Ltd - client relationship established by this document; 2. I am aware that, if my application for this loan facility is successful, I will be liable for the loan personally should the facility approved become unpaid. 3. I consent to provide proof of income, proof of residence, loan processing fees, National ID or any official ID and letter of undertaking from employer to DIGITAL MicroCredit Ltd to facilitate processing of this loan facility. 4. I consent that DIGITAL MicroCredit Ltd may make such enquiries including references to financial dealings with other financial institutions and creditors, as it considers necessary. 5. I declare that contents of this application form were clearly explained to me in the language that I understood. 6. I consent to jurisdiction of the Courts in respect of any claim arising hereunder and elect the respective address (es) above as domicilium citandi et executandi for all purposes arising from this agreement. 7. Should the loan facility be granted to me, I declare and acknowledge that the following terms will apply to such facilities: (i) Any facilities granted to me by DIGITAL MicroCredit Ltd may be cancelled at the mere notice by DIGITAL MicroCredit Ltd and any amount(s) then outstanding will be immediately due and payable, or become due and payable at the time indicated in such notice. (ii) DIGITAL MicroCredit Ltd shall charge 6% interest on reducing balance basis and delays in servicing loans shall attract a penalty of 10% of the outstanding loan arrears. (iii) The granting of facilities shall be at the sole discretion of DIGITAL MicroCredit Ltd direct to client bank account subject to terms and conditions. (iv) Digital MicroCredit Ltd shall issue a monthly updated loan statement to me. (v) I acknowledge that interest will be payable on any amounts outstanding under facilities availed. The interest charged by DIGITAL MicroCredit Ltd at its sole discretion and will be calculated on the outstanding balance and debited to the loan account on the date as DIGITAL MicroCredit Ltd may deem appropriate.
